Full online access application

Ripley Medical Centre offers online access to your medical records. This allows you to do a number of things online or via an App such as book and cancel appointments, order medication, change your preferred pharmacy, update your details and access your medical records.

Depending on which app you chose you may also be able to have Video Consultations with Clinicians at the practice, send photos and documents directly to your medical record, add reading from smart devices such as Apple Health and Google Fit and add and track readings such as weight and blood pressure. For this you would need to use Airmid UK which can be found on the App store or Google Play Store.

If you would like full access to your past records, your record will be checked by a clinician who may wish to discuss past data with you.

Please read the enclosed leaflet and consider the information carefully, and then complete this registration form and the attached questionnaire.

These will need to be returned to Ripley Medical Centre along with two forms of ID. One must be photographic and the other must show your address. Accepted forms of ID include: Passport, Driving Licence, Bank Statements. Unfortunately we cannot accept utility bills.

The practice will ask you to complete this form after the completion of the register for online services form.

  • have been requested to do so by the surgery
  • are registered at the surgery
